If you have multiple doctors that we will need to request records from, please submit a medical release authorization for each physician at the link below:

Prior to being scheduled for an appointment we must have received:

• A completed new patient packet

• A valid FL state identification

• A MRI, and/or CT scan dated within the last 2 years

• A complete list of medication for the last 6 months ( we will obtain)

• An arrest record within the last year (we will obtain)
